About this referral
The community learning disability team offers assessments and interventions to adults with a formal diagnosis of learning disability. The definition of learning disability is a significantly reduced ability to understand new or complex information (impaired intelligence), with a reduced ability to cope independently (impaired social functioning) and which started before adulthood.
This professional referral form enables you to request assessments and interventions from the health professionals within the community learning disability team, this includes:
- Learning Disability Nurses
- Nursing Assistants
- Occupational Therapists
- Physiotherapists
- Speech and Language Therapists
- Psychologists
- Assistant Practitioners
Support available includes, but is not limited to, behavioural assessments, health facilitation, capacity assessments, skills assessments, equipment assessments, communication assessments, swallow assessments, therapy programmes, mobility assessments and psychological therapies.
Who is Eligible?
Adults with a diagnosed learning disability aged 18+ and who are registered with a Bolton GP.
We have an open referral system so anyone can refer to the team, including the individual themselves, family members, carers and professionals. If referring for somebody else, consent must be obtained or considered prior to making the referral.

How long will it take?
It should take around 15 minutes to complete the referral form. Once submitted, we aim to offer an initial assessment within 4 weeks of receipt of referral. Dependent on need and service demand, referrals may be added to a waiting list for interventions.
